Use a custom Feedback link in the dashboard section
Here are the steps to do it
Step 1: Create a dashboard page
Step 2: Add a TEXT object to the dashboard or Report
Step 3: In the text object add the following script

Step 4: Check the mark as html option
Step 5: Save the dashboard
