The Forms toolbar is considered a "legacy tool." In order to access it you have to elect to display Word's Developer tab. To do this, click the Office Button, then Word Options. Click Popular, then check the box for "Show Developer tab in the Ribbon."
Once you've displayed the Developer tab, find the form fields in the Legacy Tools group of Developer
Controls.
